If this is your first appointment, please come in ten minutes early to fill out all the appropriate forms. You can also download all the registration forms on our website and complete them prior to your visit. If we are filing for your insurance, bring your insurance ID card and driver's license with you. Please be aware that some insurance companies require that a referral from your primary care physician be on file before we can schedule an office visit. We make every effort to see our patients on time, however, occasionally emergencies arise or a patient requiring more extensive care came ahead of you. Please excuse any delays. We appreciate your patience.
